How to Add Parents or Children?

The following documents are required:

  • Notarized translation of the birth certificate (no apostille required):

— If adding parents, you need to provide your own birth certificate to confirm the relationship.

— If adding children, you need to provide their birth certificates.

  • A copy of the residence permit (VKN) of the person being added (parent or child).
  • A copy of the passport of the person being added.
  • A copy of the kimlik / residence permit of the person adding the relatives (your own).
  • Medical report confirming the absence or presence of chronic diseases (kişinin durum bildirir sağlık kurulu raporu / heyet raporu):

This report is issued by appointment only at Antalya Eğitim ve Araştırma Hastanesi.

Appointments can be made by calling 182 or via the MHRS application/website.

The report is paid.

Important Information

  • Without the medical report about chronic diseases, the attached relative can use the insurance temporarily, but SGK may block access at any time.
  • SGK does not officially specify how long the insurance can be used without this report — it may be one week or one month.
  • After submitting all the documents, the attachment process is fast — you can usually use the insurance the next day.

All the listed documents are mandatory to attach parents or children to your insurance.
